When is caution advised?

Hormone therapy isn't suitable for everyone. With a history of hormone-dependent cancer, severe thrombosis or pulmonary embolism, active liver disease, or certain cardiovascular conditions, extra caution is required or treatment is contraindicated. These are individual considerations always made together with a doctor. The delivery method also matters: oral estrogens slightly increase thrombosis risk via the first-pass effect in the liver, while transdermal delivery doesn't increase that risk. That's why transdermal is the most recommended route for estrogen in current guidelines.
